The second pregnancy part V – Breech position, lockdown and Othilia becoming a big sister

The third trimester started with a week 28 appointment with my midwife. She came for a home visit and checked that everything looked good with both little brother and me. I’m monitoring my own blood pressure, blood glucose levels and looking out for protein in urea and other general things, so I updated her on that.

The next event was the week 32 appointment and this was an ultrasound to look at the growth of little brother and to check the amount of amniotic fluid, the umbilical cord and the placement of the placenta. Little brother measured completely average on all parameters and I was also doing good as long as I’m not triggering the pain in the pelvis. Everything looked good except for that little brother had turned and has since the week 32 examination been in breech position. 
For the past few weeks I have been on a mission to gently try to convince little brother to turn again to a head downward position. So far my mission has failed, but I’m hoping that he will turn again in the coming weeks. He likes to have his head stuck under my left rib cage and this is starting to get a bit uncomfortable for me. It feels like he’s pushing my lungs and stomach further up in my torso so I get easily out of breath and I can’t eat as much as I want. The 36 weeks registration at the hospital is coming up in week 37 so I will find out what their strategies are to turn little brother around.

Den tredje trimestern började med en vecka 28-undersökning med min barnmorska. Hon kom hem till oss och kontrollerade att allt såg bra ut för mig och lillebror. Jag övervakar mitt eget blodtryck, blodsockernivåer och protein i urinen och andra allmänna saker så det uppdaterade jag henne om.

Nästa händelse var vecka 32-undersökningen och där gjordes ett ultraljud för att titta på lillebrors tillväxt och för att kontrollera mängden fostervatten, navelsträngen och placentan. Lillebror mätte helt genomsnittligt på alla parametrar och min status var bra så länge jag inte utlöser smärtan i bäckenet. Allt var bra förutom att lillebror vänt sig och har sedan vecka 32 varit i sätesbjudning.
Under de senaste veckorna har mitt uppdrag varit att försiktigt försöka övertyga lillebror att åter vända sig med huvudet nedåt. Hittills har mitt uppdrag misslyckats, men jag hoppas att han kommer att vända sig igen de närmaste veckorna. Han tycker om att ha huvudet under mina vänstra revben och det börjar bli lite obekvämt för mig.  Det känns som att han skjuter mina lungor och mage längre upp i min torso så jag blir lätt andfådd och jag kan inte äta så mycket jag vill. 36 veckors-registreringen på sjukhuset kommer i vecka 37 så jag kommer att ta reda på vad deras strategier är för att vända lillebror.

Lockdown again

During this period, the third lockdown occurred in the Czech Republic. Or let’s just say that the second lockdown from October hasn’t gone away, the restrictions on human’s freedom has only become tighter now.

This takes a toll on your mental health and it is not easy to stay positive for the coming birth when everything is so uncertain. In normal times I would by now have a solid plan on where I will give birth in April, who will be there to help me through the birthing and where and with whom Othilia will be with when we are at the hospital. The answer to these questions until now is that I do not know. The hard lockdown has been extended to April 5th (the first call was that it would be until March 21st), so we’ll just have to wait and see what happens in April. This is an anxiety that I would not want to have.

It’s hard to fix everything that we need for little brother when all the physical stores are closed, but we manage anyway to slowly fill our home with baby stuff (It’s not a real pacifier Othilia has gotten in her mouth, it is for one of her dolls. Othilia has never used a pacifier.)

Under denna period inträffade den tredje nedstängningen i Tjeckien. Eller egentligen är det den andra nedstängningen från oktober förra året som inte har försvunnit, begränsningarna i människors frihet har bara blivit stramare nu.

Ens mentala hälsa tar stryk av detta och det är inte lätt att vara positiv för den kommande födelsen när allt är så osäkert. I normala tider skulle jag nu ha en gedigen plan om var jag ska föda i april, vem som kommer att vara där för att hjälpa mig igenom förlossningen och var och vem som kommer att vara med Othilia när vi är på sjukhuset. Svaret på dessa frågor fram tills nu är att jag inte vet. Den hårda nedstängningen har förlängts till 5 april (första beskedet var att det skulle bli till 21 mars), så vi får bara vänta med att se vad som händer i april. Det här är en ångest som jag inte skulle vilja ha.

We have begun the process to prepare Othilia for her upcoming role as a big sister and that soon a baby will join our family. She is only 2.5 years old so I don’t have high expectations that she understands what it means, but we think she is on board with having a baby in the family. She is very caring towards her dolls and stuffed animals and we have books about babies coming to the family that we read to her. We talk about little brother and she can point and hug my belly when we bring up little brother. One funny anecdote happened when I first read a book to her about babies in the stomach. She laughed so much in disbelief that a baby could be in mummy’s belly. Who would have figured that talking animals would be more real and accepted by her than a baby in the belly.

Vi har påbörjat processen för att förbereda Othilia för hennes kommande roll som storasyster och att det snart kommer en bebis till vår familj. Hon är bara 2,5 år så jag har inte stora förväntningar på att hon förstår vad det betyder, men vi tror att hon är ombord med att ha en bebis i familjen. Hon är mycket omtänksam mot sina dockor och uppstoppade djur och vi har böcker om bebisar som kommer till familjen som vi läser för henne. Vi pratar om lillebror och hon kan peka och krama min mage när vi tar upp lillebror. En rolig anekdot hände när jag först läste en bok för henne om bebisar i magen. Hon skrattade så mycket i misstro att en bebis kunde vara i mammans mage. Vem kunde tänka sig att talande djur skulle vara mer verkliga och accepterade av henne än en bebis i magen.

The second pregnancy part IV – Vitamin D, Corona lock down and a 3D ultrasound

I didn’t feel so good when we went into December and the Christmas month. The nausea and aversions had returned and now a great fatigue also returned. I suspected that I was not getting enough iron because I couldn’t eat that much meat, but I was too tired to go test myself. I found iron deficiency to most likely be the cause of my fatigue so I started with iron supplements and already after a week on iron supplements I felt fit enough to take me to a test site. It was not just iron levels I want to keep track of now during this pregnancy.

Jag mådde inte så bra när vi gick in i december och julmånaden. Illamåendet och aversionerna hade kommit tillbaka och nu kom även den stora tröttheten tillbaka. Jag misstänkte att jag inte fick i mig tillräckligt med järn då jag inte kunde äta så mycket kött, men jag var för trött för att gå att testa mig. Jag fann järnbrist mest sannolikt så jag började med järntillskott och redan efter en vecka på järntillskott kände jag mig tillräckligt pigg för att ta mig till ett testställe. Det var inte bara järnnivåer jag vill hålla koll på nu under graviditeten.

Week 24 examination and vitamin D

When it was time for the week 24 check-up, I had declined to do it through my obgyn clinic. It was just a general check of blood pressure and blood tests that would be done in that case and I felt that I could arrange this myself. I measure blood pressure and blood glucose levels from home, as well as protein in urine at a slightly later stage in the pregnancy, and a blood test I can do through a private company (Prevedig) where it goes quickly and smoothly (have written about the whole procedure in a previous post).
The fatigue turned out to be iron deficiency and everything else looked good so I could go into the Christmas holidays with a clear conscience.

The most important test for me was to see that I eat enough vitamin D3, it is both for protection against Corona and my immune system, but also for the baby’s development and my aging.

I have been eating 6000 IU vitamin D3 (150 micrograms) per day since the beginning of the autumn and the important thing is not the amount of supplement you eat, but what concentration you have in the blood of 25-hydroxyvitamin D3 (the metabolite of vitamin D3 that is actually transported in the blood). Both me and Pontus carry mutations that result in our livers not converting vitamin D3 to 25-hydroxyvitamin D3 in the same amount as in others. Some of the vitamin D3 we eat or get through the sun goes straight through the liver via the bile out with the feces.

I have eaten 6000 IU (150 micrograms) of vitamin D3 per day during the darker part of the year and it has been shown that this dose is good for me. I have been stable at about 42 ng/ml over this time and I’m exactly where I want to be with the concentration of 25-hydroxyvitamin D3 in the blood as the optimal dose is between 40-60 ng/ml. For protection against Corona, you should also be above 40 ng/ml and absolutely not below 30 ng/ml, so it is important to look at your levels ​​in the blood over time and adjust your intake of vitamin D3 so that it suits you. Excessive levels of vitamin D are also harmful.

När det var dags för vecka 24-kontrollen så hade jag tackat nej till att göra den via min obgyn-klinik. Det vara bara en allmän kontroll av blodtryck och blodprov som isåfall skulle göras och det kände jag att jag kunde ordna själv. Blodtryck och glukosmätningar sköter jag hemifrån, samt även protein i urin vid ett lite senare stadie i graviditeten, och ett blodprov kan jag göra via ett privat företag (Prevedig) där det går snabbt och smidigt (har skrivit om hela proceduren i ett tidigare inlägg).
Tröttheten visade sig vara järnbrist och allt annat såg bra ut så jag kunde gå in i julhelgerna med gott samvete.


Det viktigaste testet för mig var att se att jag äter tillräcklig mängd vitamin D3, det är både för skydd mot Corona och mitt immunförsvar, men också för barnets utveckling och mitt åldrande.

En del av det vitamin D3 vi äter eller får genom solen åker rätt igenom levern via gallan ut med avföringen.

För skydd mot Corona ska du även ligga på över 40 ng/ml och absolut inte under 30 ng/ml, så det är viktigt att titta på dina värden i blodet över tid och justera ditt intag av vitamin D3 så att det passar dig. För höga doser vitamin D är också skadligt.

Being pregnant during Corona lock down

For me, the Corona Restrictions have not affected me that much. Although the Czech Republic has been more or less closed since October, all visits by my midwife or to my obgyn clinic have worked smoothly and Pontus has been present on all visits. The mask restrictions are only in spaces where you can not keep more than 2 meters between each other and in our part of Prague it means we only need to use a mask in the public transportation system or in the shops. The worst thing is what a lock down is doing to the psyche and we get depressed from being trapped for this long.
The big question will be the birth as it has gone back and forth here in the Czech Republic whether one’s partner can attend or not. We will see what the situation looks like in April and I have some options depending on what any restrictions look like then. I want both my husband and my midwife with me to the birthing center so we have to see which alternative I decide to go with in the end.

This is what lock down is for us right now.

För mig har Coronarestriktionerna inte påverkat mig så mycket. Även om Tjeckien har varit mer eller mindre nedstängt sedan oktober så har alla besök av barnmorska eller hos obgynkliniken fungerat smidigt och Pontus har varit med på alla besök. Masktvånget är bara i utrymmen där man inte kan hålla mer än 2 meter mellan varandra och i vår del av Prag betyder det av vi bara behöver använda mask i lokaltrafik eller i affärer. Det som är värst  är psyket och man blir deprimerad av att vara instängd så här länge.
Det stora frågetecknet kommer vara förlossningen då det gått fram och tillbaka här i Tjeckien om ens partner kan vara med eller inte. Vi får se hur läget ser ut i April och jag har en del alternativ beroende på hur eventuella restriktioner ser ut då. Jag vill ju ha både min man och min barnmorska med mig så vi får se vilket alternativ jag bestämmer mig för i slutändan. 

The end of the second trimester and 3D ultrasound

The nausea and aversions eased again after week 24 and I was able to enjoy the last part of the second trimester. Little brother is active in the stomach now and I have been able to feel proper movements since week 18. Othilia was also calm in the stomach, but I think that little brother is calmer than her. It is rare that I feel proper kicks from him and the activity from little brother comes from the fact that he occasionally moves in his cocoon. We’ll see if he keeps that personality outside the womb as well.

I ended the second trimester with a +8 kg (17.6 lbs) on the scale, a blood pressure on its way to normal and a 3D ultrasound in week 28 to see if we could see more of little brother than we did during the week 20 ultrasound. Unfortunately, little brother didn’t want to reveal his face this time either. At least we got to see a nice heart, hands and feet, as well as an ear. Everything looked good in his development and that is the most important thing. Now there are only three months, or less than 100 days, left before we get to see him anyway.

I’ve been so bad at taking pictures during this pregnancy, so I have to make it up in writing. This is me at 27 weeks pregnant with our second child and the scale says +8 kg.

Illamåendet och aversionerna lättade igen efter vecka 24 och jag kunde njuta av sista delen av den andra trimestern. Lillebror är aktiv i magen nu och jag har kunnat känna ordentliga rörelser sedan vecka 18. Othilia var också lugn i magen, men jag anser nog att lillebror är lugnare. Det är sällan som jag känner ordentliga sparkar utan aktiviteten från lillebror kommer med från att han då och då rör sig i sin kokong. Vi får se om han behåller den personligheten även utanför livmodern.  

Jag avslutade den andra trimestern med ett +8 kg på vågen, ett blodtryck på väg till det normala  och ett 3D-ultraljud i vecka 28 för att se om vi kunde få se av lillebror än vad vi gjorde under vecka 20 ultraljudet. Tyvärr ville lillebror inte visa sig denna gången heller. Vi fick iallafall se ett fint hjärta, händer och fötter, samt ett öra. Allt såg bra ut i hans utveckling och det är vad som är huvudsaken. Nu är det bara tre månader eller mindre än 100 dagar kvar innan vi får se honom ändå.
